Risk Factors Assessment
Scoring System:
Congestive heart failure (1 point)
Hypertension (1 point)
A₂ge ≥75 years (2 points)
Diabetes mellitus (1 point)
S₂troke/TIA/thromboembolism (2 points)
Vascular disease (1 point)
Age 65-74 years (1 point)
Sc (Sex category) - Female (1 point)
CHA₂DS₂-VASc Score
0points
Interpretation:
Very Low Risk (0% yearly stroke risk): Very low stroke risk - anticoagulation generally not recommended
Treatment Recommendation
No antithrombotic therapy recommended
Consider bleeding risk (HAS-BLED score) when making anticoagulation decisions.
Risk Stratification:
• Score 0: Very low risk (0% yearly)
• Score 1: Low risk (~1.3% yearly)
• Score 2: Moderate risk (~2.2% yearly)
• Score ≥3: High risk (3.2-15.2% yearly)
Clinical Application:
The CHA₂DS₂-VASc score is the recommended tool for stroke risk assessment in non-valvular atrial fibrillation according to major guidelines (ESC, AHA/ACC/HRS).
It's more sensitive than CHADS₂ score, particularly for identifying low-risk patients. The score helps guide anticoagulation decisions but should be considered alongside bleeding risk assessment and patient preferences.
Key Points:
- • Applies to non-valvular atrial fibrillation only
- • Consider bleeding risk before starting anticoagulation
- • Regular reassessment recommended
- • Individual patient factors and preferences matter
