Bleeding Risk Factors Assessment
HAS-BLED Acronym:
Hypertension (1 point)
Abnormal renal/liver function (1 point each)
Stroke (1 point)
Bleeding history or predisposition (1 point)
Labile INR (1 point)
Elderly (age >65) (1 point)
Drugs/alcohol concomitantly (1 point)
HAS-BLED Score
0points
Interpretation:
Low Risk (1.13% yearly bleeding risk): Low bleeding risk - anticoagulation benefits likely outweigh bleeding risks
Clinical Recommendation
Anticoagulation recommended if CHA₂DS₂-VASc ≥1 (men) or ≥2 (women)
Score ≥3 suggests caution and more frequent monitoring, but is not a contraindication to anticoagulation.
Risk Stratification:
• Score 0-2: Low risk (1.02-1.88% yearly)
• Score 3: Moderate risk (3.74% yearly)
• Score ≥4: High risk (8.70-12.50% yearly)
Clinical Application:
The HAS-BLED score helps identify patients at high risk of bleeding who require more careful monitoring and consideration of modifiable risk factors when on anticoagulation.
Key Points:
- • Not intended to exclude patients from anticoagulation
- • Helps identify patients needing closer monitoring
- • Address modifiable risk factors (BP control, avoid NSAIDs)
- • Compare with thromboembolic risk (CHA₂DS₂-VASc)
- • Consider patient preferences and quality of life
Modifiable Risk Factors: Uncontrolled hypertension, labile INR, concomitant drugs (especially NSAIDs), alcohol excess.
